EXHIBIT SPACE AT THE TRADE SHOW
v  Click on the link, which will take you to the Exhibit Space Registration Page at Formsite
v  Fill out your company information
v  Read the Booth Details
v  Click on the Floor Plan to see available booth spaces
v  Click either “First Booth 8’ x 10’” or First Booth 10’ x 10’”
v  If you want any additional booths (they are at a reduced price, fill in the quantity in the box provided.
v  If you chose a booth or booths that have a premium charge, fill in the quantity of how many end cap booths you chose
v  If you are bringing additional personnel (two per booth are covered), fill in the quantity
v  Booth Selections, add your booth number(s) here (off the floor plan)
v  Representatives attending, list each first and last name in a box, spelled the way you want to see it on a badge
v  Terms and Conditions. If you want to read them all, click on the LINK titled HERE. Once you have read them and are satisfied you understand them, click in the box at the bottom to agree to the terms and conditions
v  You will see a summary of your order
v  If you want to pay by check, click the check icon now and you will get an invoice that you can use to pay from and the address on where to send the check
v  If you want to pay with either a PayPal account or another type of credit card, click on the small link that says "continue to PayPal"
v  You have just left Formsite and are now at PayPal
v  If you want to pay with a PayPal account, login
v  Fill out the form and click “Pay” – you are done!
v  You may print a receipt and the Association office will be notified of your payment
v  Then click Pay! That's it, you will get a receipt. I will be notified that you paid.
 
TRADE SHOW AND CONVENTION FULL REGISTRATION
v  Click on the link, which will take you to the Trade Show and Convention Full Registration Page at Formsite
v  Fill out your information
v  Fill in the name(s) of those attending with the names as you want to see them on a badge (click a + whenever you want to add another name)
v  Note that full registration includes a banquet ticket!
v  Let us know if you are a KTAI Member
v  Let us know if you are a First Time Attendee (Member or Non Member)
v  If you want to register for Full Tradeshow and Convention AND become a member, click the New Membership box
v  Click in whichever box indicates how many are attending
v  If you are bringing a guest that is not going to register or attend, and you want to bring them to the banquet, click on the banquet ticket boxes you choose
v  Click Continue
v  You will see a summary of your order
v  If you want to pay by check, click the check icon now and you will get an invoice that you can use to pay from and the address on where to send the check
v  If you want to pay with either a PayPal account or another type of credit card, click on the small link that says "continue to PayPal"
v  You have just left Formsite and are now at PayPal
v  If you want to pay with a PayPal account, login
v  Fill out the form and click “Pay” – you are done!
v  You may print a receipt and the Association office will be notified of your payment

TRADE SHOW ONLY REGISTRATION
v  Click on the link, which will take you to the Trade Show Only Registration Page at Formsite
v  Fill out your information
v  Fill in the name(s) of those attending with the names as you want to see them on a badge (click a + whenever you want to add another name)
v  Note that Tradeshow ONLY registration includes a badge and 1 packet, no meals or programs
v  Let us know if you are a KTAI Member
v  Let us know if you are a First Time Attendee (Member or Non Member)
v  If you want to register for Tradeshow only AND become a member, click the New Membership box
v  Click in whichever box indicates how many are attending

MEMBERSHIP DUES RENEWAL
v  Click on the link, which will take you to the Membership Dues Page at Formsite
v  Fill out your information
v  Click on the membership level you choose in the open square
v  Scroll down and click on the membership level again
v  Click Continue
v  You will see a summary of your order
v  If you want to pay by check, click the check icon now and you will get an invoice that you can use to pay from and the address on where to send the check
v  If you want to pay with either a PayPal account or another type of credit card, click on the small link that says "continue to PayPal"
v  You have just left Formsite and are now at PayPal
v  If you want to pay with a PayPal account, login
v  Fill out the form and click “Pay” – you are done!
v  You may print a receipt and the Association office will be notified of your payment

TRADE SHOW SPONSORSHIP
v  Click on the link, which will take you to the Sponsorship Page at Formsite
v  Read the sponsorship options, click continue
v  Fill out the form
v  Select your sponsorship option, click continue
v  You will see a summary of your order
v  If you want to pay by check, click the check icon now and you will get an invoice that you can use to pay from and the address on where to send the check
v  If you want to pay with a debit card, credit card or PayPal account, click the small “Continue to PayPal” text
v  You have now left Formsite and are on the PayPal site
v  If you want to pay with a PayPal account, login
v  If you want to pay with a debit or credit card, click on the “Pay with a debit or credit card, or PayPal Credit” link
v  Fill out the form and click “Pay” – you are done!
v  You may print a receipt and the Association office will be notified of your payment
 
NEW MEMBERSHIP APPLICATION
v  Click on the link, which will take you to the Membership Application on Formsite
v  Fill in the "Business Information" blanks
v  Fill in the "Business Specifics" blanks
v  Read the Membership Levels and "check" the level you wish to apply for
v  Click "continue"
v  You will see the cost of the membership level you choose
v  If you want to pay by check, click the check icon now and you will get an invoice that you can use to pay from and the address on where to send the check
v  If you wish to pay by debit card, credit card or PayPal account, click on the "Continue to PayPal" text
v  You have now left Formsite and are on the PayPal site
v  If you have a PayPal account and want to pay with that account, login
v  If you want to pay with a debit or credit card, click on the link below that says "Pay with a debit or credit card, or PayPal Credit
v  Fill in the information and click "Pay" at the bottom and you are done!
v  You may print a receipt and the Association office will be notified of your payment
v  Within 24 hours you will receive information about your membership; it will be sent to the email address you provided.
